Insurance and Billing

Performance Physical Therapy will provide the service of submitting claims to your insurer. We recommend that you become familiar with your plan so you are aware of your financial responsibilities. Performance Physical Therapy accepts patients with almost all plans. Please see below for an overview, with links to the insurance companies site.

In addition we do offer self pay rates.

Payment not covered by your insurance is due at the time of service, however if this is not possible we are happy to discuss and make a payment plan for you.
