Insurance and Billing

Performance Physical Therapy will provide the service of submitting claims to your insurer. We recommend that you become familiar with your plan so you are aware of your financial responsibilities. Performance Physical Therapy accepts patients with all types of insurance as long as you have in-network or out-of-network benefits.

 

In addition, we do offer self-pay rates. Payment not covered by your insurance is due at the time of service, however, if this is not possible we are happy to discuss and make a payment plan for you.
