In preparation for your appointment, please be aware that we have made some changes.  

 

  • We require all patients to wear a mask (unless they have a breathing problem), wash their hands upon arrival, and practice social distancing. 

 

  • All patients are screened at every visit to minimize the risk of the spread of COVID-19.  

  • Please be advised that we are unable to absorb the expenses incurred when a patient does not cancel their appointment with at least 24 hours of notice. Therefore, missed appointments and cancellations made less than 24 hours prior to an appointment will result in a $75 fee.

  • We continue to offer physical therapy through telehealth as an addition to in-clinic care or as an alternative when patients are unable to come to the clinic. 

  • To minimize contact between patients and staff, we require a credit card to be kept on file for copayments and for payment of all balances and fees associated with services rendered after claims have been processed by your insurance company. Credit card information will be retained in electronic format under the HIPAA secure merchant portal of myvirtualmerchant.com. 

  • Please wear comfortable clothing and be ready to move and feel better!
